Lower Back Disorders; Disc Herniation, Lumbalgia, Sciatic Nerve Neuralgia, Post-Isometric Relaxation & Back Rehabilitative Exercises.
Sciatic nerve neuralgia is considered to be one of the most difficult abnormalities of the support and movement system. Clinical expressions of sciatic nerve neuralgia include sharp pain irradiating to the lower extremities which increases during any of the patient's attempts to move and of course limited range of motion on all axes and planes. Usually, when a patient appears at the doctor's office with such a difficult clinical picture, in addition to prescribing painkillers, anti-inflammatory drugs, and/or utilizing injections, the doctor will usually refer the patient for radiological examination. Today, the most common radiological examinations are the MRI and/or CT scan. In many cases, MRI test detects bulging and/or herniation of discs. If symptoms are not alleviated in a period of a few weeks due to conservative methods of treatments (above mentioned oral or injected medication) consideration will be given to surgical intervention. The problem is that in many cases, this difficult neurological picture is not the result of the disc herniation, but is actually the result of piriformis muscle syndrome.
Anatomy of the Piriformis Muscles
The Piriformis muscle originates from the anterior surface of the sacrum and inserts into the greater trochanter. It shares the passage through the greater sciatic foramen with several important nervous and vascular structures including nerves which provide innervation to pelvic inner organs, the gluteals, and the lower extremities. The piriformis muscles can accumulate tension to the point that they will start compressing/ impinging sciatic nerve and awaking in some cases intolerable pain in the buttocks and lower back with irradiation to the extremities.
Sciatica nerve neuralgia can be the result of compression of the S1 spinal nerve as well as due to the compression of the sciatic nerve by over-tensed piriformis muscles. If patients with symptoms of sciatica have the ability to bend forward without awaking pain, most likely the sciatica neuralgia is the results of piriformis muscle syndrome.
The Causes for Accumulation of Tension in the Piriformis Muscles
1. Due to spondylosis, including herniation of disc, spinal nerves can be irritated (please do not confuse irritation of the spinal nerve with compression of the spinal nerve.) Irritation of the spinal nerves that provide innervation to the piriformis muscles (S1 spinal nerve) will cause a gradual accumulation of tension in these muscles, to the point that they will compress the sciatic nerve.
2. Physical overload of the muscles such as vigorous exercise, job performances that put too much static load on the piriformis muscles, hormone changes, exposure to toxins, physical and/or psychological trauma, etc.

First, let's quickly review how medical massage therapy affects the arterial blood pressure in patients with EH. There are three major mechanisms which massage practitioners should use to help patients with hypertension: Balance the sympathetic and parasympathetic divisions of the autonomic nervous system, vasodilate the vertebral arteries and reduce peripheral vascular resistance. These three mechanisms are intimately correlated, hence the need to discuss them together as parts of the same process.
Vertebral arteries arise from the subclavian arteries. They ascend through the cervical vertebrae and enter the skull where they unite to form the basilar artery, supplying the posterior part of the brain. The vertebral arteries also give off two important arterial branches that supply the entire spinal cord: The anterior spinal artery and two posterior spinal arteries. The pathway of the vertebral arteries through the cervical vertebrae is quite complex. The transverse process of each cervical vertebra has a special opening called the transverse foramen through which the vertebral artery passes. Cervical vertebrae are positioned on top of one another such that these openings form a bony canal through which the vertebral arteries ascend.
The walls of vertebral arteries have their own sympathetic plexus innervation, regulating their constriction and dilation. It follows that any irritation to this plexus may result in their contraction. Even a minor facet joint subluxation, which may not even be visible by radiographic means, can produce an irritation slightly compressing the vertebral arteries. This constriction may lead to a reduced blood supply to the brain, which in turn will cause further vasoconstriction in an attempt to compensate for compromised circulation. The result is an inevitable increase in blood pressure or EH.
Other mechanisms that may cause a decrease in blood flow through the vertebral arteries are cervical spondylosis, emotional stress and physical overload of the neck and upper back muscles. As a result of these, a hypertonus develops in the cervical muscles. In order to maintain proper function, the brain's daily perfusion has to be approximately 2,000 quarts of arterial blood. This rate is regulated by special vascular receptors in the arterial structures of the brain. Even a minor reduction in the amount of blood circulation triggers compensatory reactions such as an increased heart rate, increased cardiac output and, most importantly, an increased peripheral vascular resistance.
Peripheral vascular resistance is a major opposing force to the heart's work. Every time the left ventricle ejects blood, the force of the cardiac contraction has to overcome the resistance of arterial vessels (especially on the level of middle-sized arteries in skeletal muscles). Thus, an increased sympathetic tone triggers arteriolar vasoconstriction, which increases peripheral vascular resistance, resulting in the heart having to work harder to pump blood.

The pathogenesis of thoracic outlet syndrome can be linked with the anatomy of the anterior scalene muscle. The anterior scalene muscle starts from the transverse processes of C3-C6, slanting downward and forward, and inserts into the first rib. When it contracts, the head bends to the same side and forward.
The causes of anterior scalene muscle syndrome are:
1. Cervical spondylosis -- Spinal nerves C3-C8 provide motor innervation to the anterior scalene muscle. Their irritation or compression evokes increased tension in the anterior scalene muscle.
2. Chronic overload and muscular strain -- Chronic physical overload of the anterior scalene muscle or its strain also produce the clinical picture of anterior scalene muscle syndrome. Because the anterior scalene muscle participates in inspiration, patients with bronchial asthma frequently have anterior scalene muscle syndrome.
3. Visceral pathology -- The heart and lungs are innervated by the same segments of the spinal cord as the anterior scalene muscle. Thus, chronic cardiac and pulmonary disorders evoke reflex zones in the anterior scalene muscle and are responsible for anterior scalene muscle syndrome. Radiating pain to the left arm caused by this syndrome can also mimic heart pathology.
The brachial plexus is located between the anterior scalene and middle scalene muscles. If the tonus of the anterior scalene muscle is increased, the brachial plexus is found in the sphincter between these two muscles. As a result, the brachial plexus becomes chronically irritated and produces a rich neurological picture. The spinal nerves of C7-C8 are involved to the greatest degree (C6-C7 and C7-T1 segments of spine.)
Anterior scalene muscle syndrome causes an abundance of circulatory problems. The subclavian artery and vein pass between the anterior scalene muscle and the first rib. An over-tensed muscle can compress the artery and vein, or evoke their vasospasm by irritation of the brachial plexus. Compression of the subclavian artery produces symptoms of insufficient blood supply to the upper limb; compression of the subclavian vein produces symptoms of insufficient blood drainage (edema). Additionally, the anterior scalene muscle can compress the vertebral artery or evoke vasospasm by irritating its sympathetic plexus.

Clinical symptoms
The main clinical symptom is pain that increases: at night, during deep breathing, when the head is bent and turned to the unaffected side, and when the arm is abducted. The pain spreads to the shoulder, armpit and lateral part of the thoracic cage. Also, patients complain about numbness and paraesthesia on the ulnar edge of the hand (hypothenar and 4th-5th fingers) and arm. One of the most important diagnostic signs is diversity of vasomotor changes. These can be the result of insufficient arterial blood supply to the arm (pain in the hand especially after exposure to lower temperatures; pale, cold skin; cyanosis; weak pulse) or insufficient venous blood drainage from the arm (edemas and increased skin temperature). If anterior scalene muscle syndrome exists for a long period of time, without adequate treatment, the hand will lose its strength because of muscle atrophy mostly at the ulnar edge of the arm and the hypothenar.
An important clinical sign involves Wartenberg's test. If the examiner palpates in the spot where the anterior scalene muscle inserts into the first rib, local pain will appear. This test allows the practitioner to differentiate anterior scalene muscle syndrome can secondarily evoke rotator cuff syndrome. This is a very important diagnostic test because irritation of the brachial plexus by an over-tensed anterior scalene muscle can secondarily evoke rotator cuff syndrome. if the practitioner mistakenly starts treatment of this pathology as a rotator cuff syndrome, it will not yield significant results because the real cause of the problem is anterior scalene muscle syndrome.

Targeting HIV replication
The replication of HIV 1 is a multi-stage process.
Each step is crucial to successful replication and is therefore a potential target of antiretroviral drugs.
Step one is the infection of a suitable host-cell, such as a CD4-positive T-lymphocyte.
Entry of HIV into the cell requires the presence of certain receptors on the cell surface, CD4 -- receptors and co-receptors such as CCR5 or CXCR4.
These receptors interact with protein-complexes, which are embedded in the viral envelope.
These complexes are composed of two glycoproteins:
an extracellular gp 120 and
a transmembrane gp 41
When HIV approaches the target cell gp120 binds to the CD4-receptors. This process is termed attachment.
It promotes further binding to a co-receptor. Co-receptor binding results in a conformational change in gp120.
This allows gp41 to unfold and insert its hydrophobic terminus into the cell membrane.
Gp 41 then folds back on itself.
This draws the virus towards the cell and facilitates the fusion of their membranes.
The viral nucleocapsid enters the host cell and breaks open releasing two viral RNA-strands and 3 essential replication enzymes:
Integrase, Protease and Reverse Transcriptase.
Reverse Transcriptase begins the reverse transcription of viral RNA.
It has two catalytic domains:
The Ribonuclease-H active site
And the polymerase active site
Here single stranded viral RNA is transcribed into an RNA-DNA double helix. Ribonuclease- H breaks down the RNA.
The polymerase then completes the remaining DNA-strand to form a DNA -- double helix.
Now Integrase goes into action.
It cleaves a dinucleotide from each 3-prime end of the DNA creating two sticky ends.
Integrase then transfers the DNA into the cell nucleus and facilitates its integration into the host cell genome.
The host cell genome now contains the genetic information of HIV.
Activation of the cell induces transcription of proviral DNA into messenger RNA.
The viral messenger RNA migrates into the cytoplasm where building blocks for a new virus are synthesised.
Some of them have to be processed by the viral protease.
Protease cleaves longer proteins into smaller core proteins.
This step is crucial to create an infectious virus.
Two viral RNA-strands and the replication enzymes then come together and core proteins assemble around them forming the capsid.
This immature particle leaves the cell acquiring a new envelope of host and viral proteins.
The virus matures and becomes ready to infect other cells.
HIV replicates billions of times per day destroying the hosts` immune cells and eventually causing disease progression.
Drugs which interfere with the key steps of viral replication can stop this fatal process.
Entry into the host cell can be blocked by fusion inhibitors for example.
Inhibition of reverse transcriptase by nucleoside inhibitors or by non-nucleoside Reverse Transcriptase- inhibitors is part of standard antiretroviral regimens.
The action of Integrase can be blocked.
Protease inhibitors are also part of standard antiretroviral therapy.
Each blocked step in viral replication is a step towards better control of HIV disease.
