This episode of The Listening Post is a special show that explores some of the worst places in the world for journalism.

Every year, the French media campaign group Reporter Sans Frontieres (Reporters Without Borders) publishes its Press Freedom Index.

Looking specifically at press freedom violations, rather than broader human rights issues, the index represents an exhaustive survey of factors affecting the ability of media to operate in every country in the world, considering factors such as violence, censorship and corruption. The results make disturbing reading.

The Listening Post's Salah Khadr introduces the Index, and gives an overview of some of the 'unchanging hells' at the foot of the table.

The difficulties of reporting from some of the most closed countries that RSF describes are hard to appreciate from outside their borders. Over the last 12 months, The Listening Post's Simon Ostrovsky has travelled to a selection of the most paranoid and isolated nations. This week we present some of the 'highlights' of his trips: including Alexander Lukashenko's Soviet throwback, Belarus, General Than Shwe's junta in the jungle, Myanmar, and the restrictions still being imposed on the media in Russia.

Al Jazeera's Tony Cheng has a special report for The Listening Post from perhaps the most closed country in the world: North Korea. Despite the destabilising effects of rumours about the health of their Dear Leader, Kim Jong Il, international condemnation over their nuclear programme, and a dysfunctional economy that has brought mass starvation to its people, the DPRK maintains a status quo of near total media lockdown.

Although a trickle of outside media have been allowed in recently, as Tony's piece illustrates, what they see only offers a fleeting and superficial glimpse of life inside the so-called 'hermit kingdom'.

Once a glittering regional capital, modern Naples' reputation has been tarnished by organised crime and soaring unemployment but in this week's programme, Amanda Palmer and the 48 team meet local journalists Andrea and Ilaria, who show them how this brash city is determined to shrug off the forces holding it back.

The most destructive of these forces is the local mafia, The Camorra older and arguably more powerful than Sicily's Cosa Nostra - which operates like a vast corporation controlling Neapolitan business and local politics. But some are willing to challenge their power, as 48 discovers when Andrea takes the team to meet the The "Sott' 'o pont'" (Under the Bridge) theatre company an organisation trying to educate teenagers about the dangers of Camorrista life through drama.

For many, the most vivid image of modern Naples is the 'garbage crisis', which left the city festering in uncollected refuse for nearly a year. We head to the suburb of Villaricca, home to Europe's largest rubbish dump, to meet local percussion band Bidonvillarik. Their musical instruments are built from rubbish to draw attention to the garbage crisis - which, they say, is far from over.

Naples is proud to have maintained its own language, and nowhere is it more alive than in its beloved local folk music, Canzone Napoletana. We meet 12-year-old Fortuna, one of the genre's hugely popular child singers, as she performs to an enraptured crowd at a birthday party in one of the narrow streets of the Forcella district.

Inside Story questions the 'blame game' being played by the parties while the people of Gaza continue to suffer.

Israel has refused permission for a convoy of trucks containing basic humanitarian supplies to deliver their loads to the Gaza Strip. Initially, the authorities had said they would allow passage for the 30 truckloads of drugs and food.

As the United Nations warns that it is going to run out of food for its aid programmes, all the parties, Hamas, Fatah, Israel and Egypt are blaming one another for the mounting humanitarian crisis in the Gaza Strip.

Israel has said that as long as rocket fire continues from Gaza into Israel, the border crossings will stay closed. The UN insists that this amounts to collective punishment, which is illegal under international law.

Christopher Gunness, the spokesman for the UN refugee agency UNRWA, said: "This has become a blockade against the UN itself."

The UN is to send a convoy of food and medical supplies to help the 250,000 people displaced by recent fighting in the Democratic Republic of Congo.

Thousands of people have been displaced in eastern DR Congo amid fighting between government forces and fighters from the rebel National Congress for the Defence of the People (CNDP).

The CNDP is led by Laurent Nkunda, a renegade general who says he is fighting to protect the region's ethnic Tutsis.

The latest outbreak of violence is threatening to escalate an already disastrous humanitarian situation.

Inside Story asks why this crisis has erupted now and how much of it is fuelled by ethnic tensions and the battle to control the country's natural resources.
